Request for Issuance of Immigration Card Copy

The Consulate-General of Japan in Honolulu keeps a collection of an estimated 70,000+ resident registration cards for Japanese citizens from 1880 and on.
 
There is one registration card per household and includes the last name of the family, first names of the household members, permanent address in Japan, relationship to head of household, occupation, date of arrival in Honolulu, date of birth, registered address in Hawaii, and additional information such as date and location of death.
This information was reported and collected on a voluntary basis, so we may or may not be able to find your relatives, and the information available may not be complete.
 
In order for us to search your family, we need the following information:
  • Request form
  • Copy of your photo ID.
  • Vital records such as birth certificates for you, your parents, your grand-parents’, and so on to your immigrant relative to show proof of lineage.
